Requirements
  • Good working knowledge of Medicare Part A, certification procedures, and medical terminology.
  • 1+ years of experience in long-term care.
  • Able to work flexible hours.
  • Familiar with federal and state regulations and requirements for long-term care.
  • Must possess excellent written, oral and interpersonal skills, as well as the ability to juggle multiple priorities.
  • Must possess excellent computer skills and be knowledgeable in working with basic business computer software programs.
Responsibilities
  • Support the Director of Admissions & Social Services with meeting the health center's sales and occupancy levels in accordance with the strategic marketing plan, achieved through strategic networking with area professionals including presentations at the community, health center tours, follow-up telephone calls and appointments.
  • Obtain referrals from hospital social workers, physicians, or family members and maintain wait lists.
  • Create new and service existing relationships with professional referral sources, including telephone contact, health center tours, and in-office visits; accurately document all contact with professional referral sources.
  • Conduct admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information and medical information, attending physician information, and other pertinent and required documentation prior to admission.
  • Assist with completing health center admission agreements with new resident admissions and/or responsible party.
  • Review all admission-related paperwork with the resident and/or responsible party including Medicare Part A coverage and non-coverage procedures; maintain all non-medical record related paperwork in an admissions file for each resident.
  • Perform meet and greets and room orientation, including presentation of a Welcome Gift.
  • Assist with maintaining the Inquiry Log (e.g., Salesforce) on a daily basis to ensure completion and accuracy.
  • Maintain Life Care resident statistics, permanent and temporary assignments, and notify resident and/or the responsible party of permanent and temporary status in the health center.
  • Help coordinate utilization review for new admissions covered by Medicare Part A.
  • Maintain complete confidentiality regarding admission information and resident records.
  • Assist in coordinating admissions/transfers/discharges with other departments.
  • Ensure that the resident, family, and responsible person are fully informed of the resident’s personal and property rights.
Desired Qualifications
  • Associate’s degree is preferred in administration or social services.
  • Case management experiences a plus.
