Requirements
  • Graduate of an approved school of nursing.
  • Must hold and maintain a current Florida Registered Nurse license.
  • Three years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
  • Demonstrated organizational skills, excellent verbal and written communication skills, ability to lead and coordinate activities of a diverse group of people in a fast-paced environment, critical thinking and problem-solving skills, and computer literacy.
Responsibilities
  • Assess patients upon admission and concurrently for discharge planning needs, medical necessity, risk factors, anticipated needs, expected length of stay, appropriate level of service, and barriers to care or discharge.
  • Develop and implement individualized care transition and discharge plans.
  • Coordinate care with physicians, nurses, social workers, therapists, ancillary services, patients, families, caregivers, third-party payers, and other interdisciplinary stakeholders.
  • Ensure safe, timely transitions to post-acute care, including skilled nursing facilities, rehabilitation, home health, hospice, and other appropriate levels of care.
  • Monitor and manage length of stay, identify potentially avoidable days, and address barriers to care early.
  • Perform medical necessity screening, submit cases for Physician Advisor review, assign initial diagnosis-related groups to determine geometric mean length of stay, and apply InterQual guidelines.
  • Participate in payer communication, authorization processes, concurrent dispute management, and review requirements for managed contracts, Medicare, Medicaid, and campus-related admissions and continued stays.
  • Lead and facilitate multidisciplinary patient care conferences and Complex Case Review.
  • Make appropriate referrals, including referrals of complex patients to Social Work Services.
  • Educate physicians, patients, families, and caregivers about care plans, procedures, therapies, treatment plans, discharge plans, and available options.
  • Promote informed decision-making and address social determinants of health and cultural, socioeconomic, religious, and other factors affecting treatment and recovery.
  • Maintain accurate, organized, retrievable, and confidential documentation supporting medical necessity, care coordination, regulatory compliance, reimbursement, and work performed.
  • Ensure compliance with federal, state, local, Joint Commission, Orlando Health, and Utilization Management Plan requirements.
  • Integrate utilization management, transition management, and care coordination standards to support denial prevention, readmission prevention, patient satisfaction, throughput efficiency, and appropriate care sequencing.
  • Prioritize assigned activities for high-risk, high-cost, and problem-prone cases while managing multiple priorities.
  • Monitor and evaluate data, fiscal outcomes, and other relevant information to develop and implement improvement strategies.
  • Forward identified quality and risk issues appropriately.
  • Maintain relationships with outside and onsite reviewers and payer representatives.
  • Review discharge plans at multidisciplinary meetings or staffing to facilitate communication with healthcare team members.
  • Participate in quality improvement initiatives, committees, task forces, educational programs, workshops, and professional development activities.
  • Maintain confidentiality of patient and staff information and comply with applicable privacy, attendance, and organizational policies.
  • Review current literature and maintain updated reference materials and reimbursement information.
  • Perform other assigned or required duties.

Florida Cancer Specialists & Research Institute has opened a new 31,000-square-foot facility at Orlando Health Institute Square in St. Petersburg, expanding cancer care access in Pinellas County.
