PATH is seeking experienced social services candidates with leadership aptitude to join our Multidisciplinary Outreach Team in San Diego as a Clinical Outreach Specialist.
ABOUT THE PROGRAM
PATH’s Multidisciplinary Team (MDT) serves the most vulnerable unsheltered individuals in the City of San Diego, using a “do whatever it takes” approach to develop trust and rapport through persistent and frequent engagement. The program provides high intensity case management services with the goal of connecting individuals to services that will ultimately end their homeless episode and obtain permanent housing
WHAT IS A CLINICAL OUTREACH SPECIALIST?
The Clinical Outreach Specialist serves on the City of San Diego Multidisciplinary Outreach Team (MDT). The position works as part of a multidisciplinary team that includes a peer support specialist, housing specialist, and street health team. Responsibilities include:
- Coordinate intake and needs assessment for all clients and work with clients to develop Housing Stability Plans (HSP) that address barriers to obtain services/housing and/or perform a warm handoff to in-house case manager or another homeless service provider that will be the primary for case management services.
- Complete biopsychosocial assessments that drive the intervention and supportive services.
- Perform persistent and frequent outreach services, contacting homeless persons in all places where they congregate in the geographic areas covered under our contracts.
- Appropriately and professionally advocate for client’s needs. This includes mediating disputes between homeless persons and community members/neighborhood stakeholders.
- Provide supportive services in a non-judgmental manner.
- Drive the outreach vehicle and transport clients to appropriate services.
- Crisis intervention for individuals experiencing homelessness in the community.
- Monitor and evaluate client’s progression through their Individual Service Plan (ISP) and develop modifications to the plan as necessary.
- Complete case notes and data entry into the Homeless Management Information System
- Become knowledgeable of all pertinent services in the community that meet the needs of the individuals served. Provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
- Attend community meetings. Collaborate and communicate with internal departments and external partnering agencies.
WHAT YOU BRING
- Computer skills with proficiency in Microsoft Office. Experience using a data entry system, and/or customer relations management (CRM) system, and/or electronic health record (EHR).
- Demonstrated knowledge of and experience with advanced and evidence-based interventions such as, but not limited to:
- Critical Time Intervention
- Harm Reduction
- Crisis Intervention
- Motivational Interviewing
- Proficiency with computers and technologies used for work outputs including:
- Microsoft Office Software
- Database Systems
- A non-judgmental and respectful approach when working with individuals who present for services with urgent and multiple case management and health needs.
- Professionalism and mature judgement.
- High level of motivation and self-starting demeanor.
- Demonstrated ability to effectively resolve and cope with immediate crisis situations.
PREFERRED QUALIFICATIONS
- An active clinical license in social work, marriage and family therapy, or professional clinical counseling
- HMIS experience.
- Experience working with vulnerable populations.
MINIMUM QUALIFICATIONS
- Hold a Master’s Degree in psychology, social work, sociology or a related field
MANDATORY REQUIREMENTS
- Be able and willing to provide in-home and community-based services on a regular basis
- Be able and willing to work flexible hours which may include evenings or weekends
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
