WellSky has expanded its CarePort Care Transitions Dashboard Suite with new intelligence dashboards designed to improve visibility into referral activity and operational bottlenecks. The enhanced reporting tools are embedded within WellSky's care management workflows and provide interactive insights to help hospital care teams evaluate performance and identify opportunities to improve patient outcomes. The dashboards leverage data from WellSky's network, which connects over 2,000 hospitals and 120,000 post-acute and community providers. They enable care teams to track patient patterns across networks, identify discharge delays, and compare performance against peer organisations using national network data. The new tools address the challenge of fragmented reporting by embedding visual insights directly into daily workflows. WellSky serves more than 20,000 client sites across the US healthcare system.
WellSky has launched the WellSky Point of Care app for Home Health, a mobile-first application that integrates AI-driven insights with the full patient record to support clinicians during home health visits. The app is included for all current WellSky Home Health EHR clients. Built on WellSky's SkySense AI platform, the app features AI-powered ambient documentation, pre-visit summaries, medication reconciliation, wound care assessments, and a conversational assistant that surfaces relevant information in real time. The technology builds on WellSky Scribe, which has reduced charting time by up to 50% across over 400 home health agencies. The application consolidates all point-of-care workflows onto clinicians' phones or tablets whilst maintaining clinician control over care decisions. WellSky serves more than 20,000 client sites globally.
WellSky has expanded its partnership with Unison Health Services, a Los Angeles home health agency serving approximately 3,200 patients. Unison is deploying WellSky SkySense AI, including AI-powered ambient documentation, to reduce clinician burden and improve workforce retention. In five months, 35 Unison clinicians documented over 5,400 visits using the technology, reporting time savings of 15 to 20 minutes per chart. The agency plans to expand the tool across all clinicians and additional visit types. A WellSky client since 2014, Unison adopted the AI suite as part of its clinical innovation strategy. CEO Stephanie Carpentier said the technology has become essential for staff, with clinicians saying they couldn't imagine working without it, driving both retention and care quality.
WellSky has partnered with Google Cloud to integrate AI capabilities into its CarePort solutions, creating an AI-powered provider directory for hospital discharge planning. The system uses Google Cloud's Vertex AI, Gemini models and BigQuery to continuously update provider data, reflecting real-time changes in services, insurance acceptance and coverage areas. The technology addresses the challenge of maintaining current provider directories, which hospitals use to guide post-acute care discharge decisions. By automating data validation, the system reduces manual verification needs and helps hospitals match patients with appropriate post-acute providers based on real-time availability and expertise. WellSky serves more than 20,000 client sites, including hospital systems, home health franchises and post-acute providers. The company will showcase the new capabilities at the ACMA National Conference in Orlando from 20-23 April.

The platform includes three key features: WellSky Extract, which uses AI to capture information from documents and populate EHR data, saving an average of 30 minutes per admission; WellSky Scribe, which provides ambient listening and transcription that has reduced documentation time by up to 50% in home health settings; and WellSky Summarize, which generates medical record summaries for quality assurance and risk identification. The expansion supports WellSky's strategy to help providers adapt to value-based care models through improved data structure and operational efficiency.
