Requirements
  • Graduate of an accredited Doctor of Pharmacy program
  • Must obtain unrestricted licensure to practice as a Pharmacist in the State of Tennessee by July 31st of their residency year
  • Submit application in the PhORCAS application portal by January 2nd
  • Complete an online application with Vanderbilt University Medical Center for the VSP PGY1 Community-Based Pharmacy Residency
  • Submit a CV, Letter of Intent, three letters of recommendation (minimum of two letters from clinical preceptors), Immunization Course Certificate of completion and University transcripts for pharmacy coursework via the ASHP PhORCAS application system
Responsibilities
  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy
  • Provides outstanding patient care by optimizing medication access navigation, prescription fulfillment, medication education and resolving medication-related problems
  • Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas
  • Develop specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients
  • Assists with clinical on-call and weekend call center coverage as scheduled
  • Contribute to and completes residency projects associated with quality, safety and/or service-line enhancement to improve delivery of care to patients
  • Completion of residency requirements during the 52 week training period as stated in the program specific residency manual
