Job Summary:
Vanderbilt University Hospital Post‐Graduate Year One (PGY1) Community-Based Pharmacy Residency program is a minimum 52 weeks, full-time residency, accredited by the American Society of Health-System Pharmacists (ASHP) in partnership with American Pharmacists Association (APhA). The residency program builds upon Doctor of Pharmacy (PharmD) education and outcomes to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives. Residents who successfully complete the residency program will be skilled in diverse patient care, practice management, leadership, clinical research and education, and be prepared to provide patient care, seek board certification in pharmacotherapy (i.e., BCPS), and pursue advanced education and training opportunities including postgraduate year two (PGY2) residencies. The resident pharmacist must complete residency requirements specified in the program residency manual that demonstrates pharmacist proficiency and contribution to projects related to the strategic goals of the department and institutions.
Residents are expected to obtain unrestricted licensure to practice in the State of Tennessee by July 31st of their residency year. Residents who are not fully licensed by this date may have modifications to their scheduled responsibilities. Failure to obtain licensure by October 1st will result in termination from the residency program and termination from employment at VUMC, unless licensure is anticipated within 120 days after the program start date. A final written warning will be issued to the resident if not fully licensed within the resident’s first 90 days of employment. Verification of licensure occurs in accordance with VUMC policy (Pharmacy Policy - Pharmacy Residency Program Standards v.6 (policytech.com)). Until residents obtain unrestricted licensure to practice in the State of Tennessee, all job functions will be supervised by a licensed pharmacist.

KEY RESPONSIBILITIES
• Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications. Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
• Provides outstanding patient care by optimizing medication access navigation, prescription fulfillment, medication education and resolving medication-related problems.
• Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
• Develop specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient.
• Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
• Assists with clinical on-call and weekend call center coverage as scheduled.
• Contribute to and completes residency projects associated with quality, safety and/or service-line enhancement to improve delivery of care to patients.
• Completion of residency requirements during the 52 week training period as stated in the program specific residency manual.

TECHNICAL CAPABILITIES
• Medication Management (Beginner): Demonstrates proficiency in common and high volume prescription/therapy management. Has the ability to determine the clinical appropriateness of each medication taken by the patient, determine the appropriateness of the dose, dosage form, indication, contraindications, potential adverse effects and potential problems with concomitant medications. Able to monitor for therapeutic duplication or other unnecessary medications, interpret, monitor and assessing patient's laboratory results as they relate to medication therapy and provide drug information to other healthcare professionals, patients and their families.
• Pharmaceutical Drug Preparation & Dispensing (Intermediate): Possesses sufficient fundamental proficiency to successfully review, prepare and dispense prescribed medications.
• Pharmaceutical Quality Assurance & Compliance (Beginner): Demonstrates knowledge of the appropriate rules and regulations as they apply to the organization. Demonstrates proficiency in process improvement, quality assurance practices and/or programs. Demonstrates a commitment to continuous quality improvement and ensures compliance with a federal, state and local regulations as well as organizational policies and procedures.
• Pharmaceutical Computer Literacy (Beginner): Has demonstrated a proficient understanding of computers or has successfully completed training for applications that are used to support medication use processes. Is capable of using the fundamental features of pharmaceutical computer applications to support patient care issues of moderate difficulty. Has proficiency in office management applications: email, calendar scheduling, messaging, phone, video conferencing and simple databases (ex: excel).
Position Qualifications (Required):
Applicants must submit their application in the PhORCAS application portal by January 2nd and complete an online application with Vanderbilt University Medical Center for the VSP PGY1 Community-Based Pharmacy Residency. A CV, Letter of Intent, three letters of recommendation (minimum of two letters from clinical preceptors), Immunization Course Certificate of completion and University transcripts for pharmacy coursework are required to be submitted via the ASHP PhORCAS application system.
Applicants will be required to be a graduate of an accredited Doctor of Pharmacy program and must obtain unrestricted licensure to practice as a Pharmacist in the State of Tennessee by July 31st of their residency year. Residents who are not fully licensed by this date will have modifications to their scheduled responsibilities. Failure to obtain Tennessee licensure by October 1st will result in dismissal from the residency program and termination from employment at VUMC, unless licensure is anticipated by October 31st. Verification of professional degree conferred, as well as Tennessee Pharmacist licensure occurs in accordance with VUMC policy.

VUMC will conduct pre-employment background checks on all final candidates for all positions in compliance with all federal, state, and local laws. The background check process will begin after a contingent offer of employment is made and accepted by the selected/matched residency candidate. Pharmacy employees and candidates who are conditionally offered employment at Vanderbilt University Medical Center will also be tested for compliance with alcohol and drug use in accordance with VUMC policy.
