Requirements
  • Missouri or Compact Registered Nursing licensure is required.
  • A valid driver's license is required.
  • A vehicle with current auto insurance is required.
Responsibilities
  • Manage a caseload, including supervision of Licensed Practical Nurse visits.
  • Conduct routine visits, evaluations, and starts of care as part of a varied weekly caseload.
  • Provide patient education on congestive heart failure, chronic obstructive pulmonary disease, diabetes, wound care, and medication management.
  • Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
  • Coordinate services from healthcare providers and communicate with health insurance companies.
  • Complete initial physical assessments and develop personalized care plans aligned with physician orders.
  • Work alongside physical therapy, occupational therapy, and speech therapy teams.
Desired Qualifications
  • Home health or critical care experience is preferred.
