Requirements
  • A master's degree in psychology, social work, psychosocial rehabilitation, or a related human services field is required, along with eligibility for a New Hampshire license and third-party reimbursement.
  • Competence in individual and group psychotherapy is required.
  • Familiarity with severe and persistent mental illnesses is required.
Responsibilities
  • Provide comprehensive biopsychosocial assessments and determine eligibility for Older Adult Program services.
  • Complete Functional Impairment Assessments and Consumer Certifications for Eligibility for Community Support Program case management consumers.
  • Develop and renew individualized service plans cooperatively with consumers and other appropriate parties.
  • Develop Individualized Wellness and Recovery Plans with consumers to help manage emerging crises and identify resources for maintaining community living during periods of crisis.
  • Facilitate and coordinate individual treatment planning to maximize the impact of services and treatment.
  • Provide individual or group counseling and psychotherapy when recommended and included in the consumer's individualized service plan.
  • Monitor referrals to specialized treatment options and participate, when possible, in the interdisciplinary teams of those providers.
  • Participate in delivering individual and group mental illness management services and case management services.
  • Maintain consumer case records and documentation accurately, timely, and professionally.
  • Complete other documentation required by the organization.
  • Maintain appropriate automobile insurance coverage for vehicle use in performing job duties.
  • Comply with organizational policies and participate in required training.
  • Maintain professional and ethical conduct.
  • Meet productivity goals and all compliance requirements.
  • Participate in peer and group supervision and administrative team meetings.
