Requirements
  • A high school diploma or GED is required.
  • At least two years of work experience in the human services field or a related area is required.
  • Solid knowledge of community resources and services is required.
  • Excellent verbal and written communication skills and good presentation skills are required.
  • The ability to work independently and as part of a multidisciplinary team is required.
  • Strong computer skills and knowledge of Microsoft Office applications are required.
  • The ability to conduct fieldwork and travel in New York City up to five days per week is required.
  • The ability to travel via public transportation is required.
  • The ability to lift 25 pounds of outreach flyers and materials is required.
  • The ability to work occasional events outside of business hours is required.
Responsibilities
  • Conduct program outreach and community-related activities to increase the referral base and community visibility.
  • Provide presentations to networks and further develop referral sources.
  • Promote linkages with service providers to help participants obtain and maintain housing stability.
  • Participate in consortium and community meetings across Manhattan.
  • Conduct initial prescreening and assessments for prospective program participants.
  • Maintain logs of outreach activities.
  • Complete monthly reports as required.
  • Cultivate, develop, and maintain productive relationships with referral agencies.
  • Attend interdisciplinary team meetings, case conferences, meetings, and outside events as assigned by the supervisor.
  • Support engagement efforts with landlords and other partners while cultivating new housing resources for Homebase clients.
  • Update the resource book for staff.
  • Promote linkage development and monitor the effectiveness of partnerships with service providers.
  • Participate in internal and external training programs and committees.
  • Follow up with households to assess their ongoing employment and housing stability needs.
  • Assist program management with regular participant record reviews.
  • Document all services in compliance with agency standards.
  • Participate in individual supervision, team meetings, case conferences, division meetings, and trainings.
  • Respond to information requests from internal S:US staff.
  • Assist with accreditation reviews, monitoring visits, and program audits.
  • Respond promptly and accurately to stakeholder needs and represent the agency appropriately.
  • Complete other duties as assigned by management.
Desired Qualifications
  • Working knowledge of homelessness prevention services is preferred.
  • Commitment to the mission and values of S:US and understanding of the issues facing the people served are preferred.

S:US helps New Yorkers who face homelessness, disability, behavioral health and addiction challenges, poverty, criminal justice involvement, and trauma access a wide range of supports across New York City and Long Island. It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
