Requirements
  • Requires a Master of Arts or Master of Science degree in social work, counseling, or a related behavioral health field, or a degree in nursing, with a minimum of 3 years of clinical experience in social work counseling involving a broad range of complex psychiatric and substance use disorder treatment; equivalent education and experience may be considered.
  • Maintain a current, active, unrestricted license such as Registered Nurse, Licensed Clinical Social Worker, Licensed Mental Health Counselor, Licensed Independent Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, or Clinical Psychologist to practice as a health professional in Connecticut.
  • Have previous experience in case management and telephonic and/or in-person coaching with members who have a broad range of complex psychiatric, substance use, and/or medical disorders.
  • Have managed care experience.
Responsibilities
  • Perform case management telephonically within the scope of licensure for members with behavioral health and substance use disorder needs.
  • Provide telephonic behavioral health resource navigation, care coordination, and referral support through the ACCESS for Adults program.
  • Partner with providers, patients, and community organizations to identify behavioral health and community-based resources that meet each individual's clinical, geographic, insurance, and personal needs.
  • Support individuals in overcoming barriers to care by coordinating referrals, assisting with appointment navigation, and conducting follow-up outreach to promote successful connection to recommended services.
  • Respond to ACCESS for Adults referrals by identifying resources, coordinating appointments, conducting follow-up outreach, and maintaining current knowledge of behavioral health, substance use, and Connecticut community resources.
  • Use appropriate screening criteria knowledge and clinical judgment to assess member needs.
  • Conduct assessments to identify individual needs and develop specific care plans addressing objectives and goals identified during assessment.
  • Support member access to appropriate, quality, and cost-effective care.
  • Coordinate with internal and external resources to meet identified member needs and collaborate with providers.
  • Serve as a resource to other Behavioral Health Case Managers.
Desired Qualifications
  • Dual licensure in mental health and substance use treatment.
  • Knowledge of Connecticut behavioral health, substance use, and community-based resources.
  • Experience providing telephonic care coordination, resource navigation, or referral services.
  • Ability to identify appropriate referrals based on clinical needs, insurance coverage, geographic location, provider availability, and individual preferences.
  • Communication, motivational interviewing, and customer service skills.
