  • A Bachelor’s degree in Social Work is required.
  • Must hold and maintain an active Social Work license in good standing in accordance with applicable state licensing requirements.
  • At least one year of supervised social work experience in a healthcare setting working directly with individuals is required.
  • Ability to build and maintain professional relationships and collaborate with external agencies to address health and welfare needs.
  • Ability to work effectively with diverse populations in community and long-term care environments.
  • Strong critical thinking skills with the ability to evaluate case information and make informed decisions regarding appropriate interventions and procedures.
  • Prior experience in long-term care is required.

Responsibilities
  • Assist with the pre-admission and admission process and help clients adjust to nursing home placement.
  • Interview residents and families to obtain relevant past and present adjustment, behavior, and lifestyle information.
  • Document progress and update plans of care at interdisciplinary care conferences.
  • Arrange transportation, make appointments, and act as a liaison between departments, residents’ families, and outside agencies.
  • Assist with handling and accepting placement through one-on-one counseling and group work.
  • Assess potential discharge needs and develop discharge plans when clients are admitted.
  • Arrange post-discharge services and follow-up care.
  • Assess the emotional, financial, spiritual, educational, and social needs of residents and ensure those needs are addressed by the facility or outside agencies.
  • Act as a resident and family advocate and ensure residents understand and exercise their rights.
  • Organize and conduct Family Council Meetings and maintain meeting minutes.
  • Maintain effective communication with residents, families, healthcare professionals, and facility staff.
  • Assist with training and onboarding new hires.
  • Perform other job duties as assigned by the supervisor.
