- An active Registered Nurse license in the state in which the candidate practices, or new graduates must be eligible to take the National Council Licensure Examination.
- Conduct Start of Care comprehensive assessments and complete OASIS documentation timely.
- Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
- Prepare accurate, orderly, and timely documentation.
- Coordinate care with patients and provide proper reports to the Case Manager.
- Provide health care instructions to patients and families as appropriate per assessment and plan.
