- An active Michigan occupational therapy license in good standing is required.

- Ability to carry and transport basic therapy equipment as needed is required.
- Competency in core home health occupational therapy scope, including activities of daily living and instrumental activities of daily living training, home safety, adaptive equipment, and caregiver education, is required.
- Complete in-home occupational therapy evaluations and deliver skilled occupational therapy visits.
- Build and execute plans of care focused on activities of daily living, instrumental activities of daily living, safety and fall prevention, and functional independence.
- Recommend and train patients and caregivers on adaptive equipment and home modifications as needed.
- Coordinate with nursing and the broader care team when a patient's condition or needs change.
- Document promptly and thoroughly in Homecare Homebase using defensible, efficient, and consistent practices.
- Carry a high-quality caseload.
- Own patient outcomes by driving low avoidable-hospitalization rates and strong patient-experience scores and participating in case conferences and team huddles to improve quality.
- Build trust with referral sources through responsive and excellent patient care.
- Help establish clinical, documentation, cadence, and professional-judgment standards as the care pod grows.
- Leverage software to accelerate charting and provide feedback to improve the platform.
- Pick up patient visits based on availability on weekdays and/or weekends as needed.
- Home health experience is strongly preferred.
- Prior home health experience is preferred.
- Experience with Homecare Homebase or a similar home health electronic medical record system is preferred.
- Deeper experience with occupational therapy populations such as neurological conditions, cerebrovascular accident, fall risk, and dementia or caregiver training is preferred.
- Strong organization and time-management skills are preferred.
