- Applicants must be licensed Nurse Practitioners or Physician Assistants and eligible for Vermont licensure.
- Nurse Practitioners must graduate from a Nurse Practitioner program with a Master's or Doctoral degree approved by the Vermont Board of Nursing or a national accrediting agency.
- Nurse Practitioners must maintain active registered nurse licensure and active, unrestricted Advanced Practice Registered Nurse licensure through the Vermont Board of Nursing.
- Nurse Practitioners must have Drug Enforcement Administration registration or eligibility for registration.
- Physician Assistants must successfully complete a Physician Assistant program at an institution accredited by the Accreditation and Review Commission on Education for the Physician Assistant.
- Physician Assistants must hold a Bachelor's degree.
- Physician Assistants must hold current certification by the National Commission on Certification of Physician Assistants.
- Physician Assistants must have active, unrestricted licensure to practice as a Physician Assistant through the Vermont Board of Medical Practice.
- Physician Assistants must have Drug Enforcement Administration registration or eligibility for registration.
- Provide follow-up care for patients with chronic pain conditions, particularly non-surgical spine patients.
- Perform triage and referral evaluation to determine appropriate care pathways, including orthopedic spine consultation, pain management or procedural interventions, and referral back to primary care.
- Coordinate care across multidisciplinary teams including Anesthesiology and Orthopedics.
- Support follow-up and care planning.
- Collaborate closely with physicians and existing advanced practice providers in the orthopedic group.
- Contribute to improving patient access, continuity, and the overall care experience.
- Prior advanced practice provider experience in spine, orthopedics, neurology, or pain management.
- Strong clinical assessment and triage skills.
- Ability to manage complex chronic pain patients.
- New graduates with relevant training or a strong interest are encouraged to apply.
- Registered Nurse First Assistant certification is strongly recommended.
- A Master's degree is preferred for the Physician Assistant pathway.
