With the vision to create a just and equitable community that is healthy, safe, economically secure, and free of systemic racism, Aging and Disability Services Division (ADSD) is seeking a Human Services Clinician II in the Adult Protective Services (APS) and Adult Services (AS) units. This position provides care coordination, case management, and supportive services to adults aged 60 and older and adults aged 18 and older with permanent disabilities.

The APS and AS units work to prevent abuse, neglect, and exploitation; ensure individuals can remain safely in their homes whenever possible; and connect clients to internal and community-based resources. ADSD operates within an integrated social and wellness model, offering wraparound support such as home-based nursing case management, in-home services, Medicare counseling, senior adult mental health services, and volunteer guardianship.

Minimum:

• Master’s degree in Human Services, Counseling, Psychology, or other directly related clinical discipline that required a practicum involving case management and/or treatment services; and
• Two (2) years of post-master’s degree experience providing professional clinical human services to older adults, including case management and/or treatment services.

Substitution: Directly related higher level clinical degrees may substitute for the Master's Degree education requirement and 1 year of experience.

Desirable: Preference may be given to applicants with experience in one or more of the following:

• Providing services to persons with disabilities, older adults (60+), and their families;
• Working in Adult Services, APS, individuals with SMI and/or experiencing homelessness, or other case management practice;
• Collaborating with Courts and the legal system when providing services; and/or
• Serving culturally diverse populations and demonstrating strong independent judgment and critical thinking skills.

Special Requirements

Background Check: A pre-employment criminal records check will be conducted. It will include checks of the following: criminal record, driving record, education, professional licensure, and credit history. You will be required to sign a release authorizing the County to obtain your background information.

Driver's License: Applicant must possess, or obtain by time of appointment, a valid driver’s license, and must provide or authorize Arlington County to obtain a copy of official driving record. Any offer of employment may be contingent upon favorable review of the applicant’s driving record. Alternatively, the applicant must have the ability and willingness to use alternative methods of transportation to perform assigned duties and responsibilities at locations other than the primary worksite.

Additional Information

Work Hours: Monday through Friday, 8:00 am to 5:00 pm. Some evening and weekend work may be required. Limited telework and remote working will be permitted in accordance with job duties.

Specific duties include:

• Maintaining a caseload of clients who meet the DARS services guidelines and who are at risk of abuse, neglect, and exploitation;
• Performing comprehensive assessments using the Virginia Uniform Assessment Instrument (UAI) to assess physical and mental health, cognitive functioning, financial situation, and formal/informal support systems;
• Collaborating with clients to develop person-centered, trauma-informed, solution-focused care plans to prevent or reduce abuse, neglect, or exploitation;
• Evaluating progress on care plan objectives, modifying plans as needed, and closing cases when objectives are achieved;
• Working with multidisciplinary teams to address unmet needs, stabilize crises, and engage family/caregivers in care planning;
• Assessing clients at risk of abuse, neglect, and exploitation and implementing care plans to improve safety;
• Participating in court processes, including but not limited to guardianship and conservatorship hearings;
• Offering alternatives to living environments when appropriate, including placement in long-term care residences;
• Ensuring effective and timely documentation of all case notes and records; and
• Maintaining electronic case records in compliance with applicable clinical and regulatory requirements.
