Requirements
  • Licensed Registered Nurse in good standing for the State of Michigan

Responsibilities
  • Triage calls and coordinate care as needed
  • Collaborate with primary care practices, specialists, and other members of the health care team to ensure the delivery of quality, efficient, and cost-effective health care services
  • Follow evidence-based clinical guidelines, preventive guidelines, and protocols in care delivery to the patient
  • Coordinate care across acute and other ambulatory settings and help patients/families understand health care options
  • Connect patients with Acute Care Managers and other resources as needed
  • Demonstrate the ability to set appropriate boundaries with patients and families
  • Demonstrate ability to implement evidence-based interventions and protocols for chronic conditions

Desired Qualifications
  • Bachelor degree (BSN or BS) preferred
  • Experience in ambulatory care is desirable
  • Experience in acute care or post-acute care setting is desirable
