- Licensed Registered Nurse in good standing for the State of Michigan
- Ability to utilize word processing, spreadsheet, presentation programs, databases, and other software relevant to the job

Responsibilities
- Triage calls and coordinate care as needed
- Collaborate with primary care practices, specialists, and other members of the health care team to ensure the delivery of quality, efficient, and cost-effective health care services
- Follow evidence-based clinical guidelines, preventive guidelines, and protocols in care delivery to the patient
- Coordinate care across acute and other ambulatory settings and help patients/families understand health care options
- Connect patients with Acute Care Managers and other resources as needed
- Demonstrate the ability to set appropriate boundaries with patients and families

Desired Qualifications
- Bachelor degree (BSN or BS) preferred
- Experience in ambulatory care is desirable
- Experience in acute care or post-acute care setting is desirable
