Requirements
  • A Master’s Degree or commensurate experience and satisfactory completion of Nurse Practitioner licensure.
  • Current Nurse Practitioner licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
  • Board certification from the American Association of Nurse Practitioners, American Association of Critical-Care Nurses, or American Nurses Credentialing Center in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current Basic Life Support, Advanced Cardiovascular Life Support, or cardiopulmonary resuscitation certification.
  • At least one year of Nurse Practitioner experience.
  • Ability to travel up to 80–90% of the time for extended periods, including overnight travel and weekends across the assigned state and possibly neighboring states.
  • A valid state driver’s license for the state of residence.
  • Automobile insurance coverage meeting state requirements, including bodily injury coverage of $100,000 per person and $300,000 per incident, property damage coverage of $50,000, and collision and comprehensive deductibles of $1,000 or less.
  • Strong computer skills and familiarity with employee health or medical record software.
  • Excellent verbal and written communication skills.
Responsibilities
  • Conduct adult and geriatric assessments covering medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Administer and order point-of-care and laboratory-based testing as needed.
  • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with the primary care physician on patient education and follow-up as necessary.
  • Provide services at members’ residences, including homes and skilled nursing facilities.
