Requirements
  • Provide patient care under the direct supervision of a licensed nurse and comply with organization policies, procedures, training, and applicable regulations.
  • Complete CMS-approved state and/or national certification requirements within the required state or CMS timeline.
  • Complete all applicable state licensure, education, and training requirements.
  • Successfully complete the organization dialysis training program and CPR certification.
  • Pass the Ishihara Color Blindness test as a condition of employment, subject to reasonable accommodation consideration.
  • Perform direct patient care involving frequent standing, bending, heavy lifting and moving of patients, assisting with ambulation, and lifting solutions and equipment within the stated weight limits.
  • Demonstrate commitment to the organization's culture, values, and customer service standards.
Responsibilities
  • Provide safe and effective dialysis therapy as part of the dialysis healthcare team under licensed-nurse supervision.
  • Set up and operate hemodialysis machines and perform required pretreatment alarm testing, including Pressure Holding testing.
  • Provide patient-specific education regarding adequacy measures, disease processes, vascular access, treatment options, and home dialysis under appropriate supervision.
  • Welcome assigned patients, inquire about their wellbeing, evaluate patients before and after treatment, and monitor their response to dialysis therapy.
  • Evaluate vascular access for patency, perform cannulation, monitor arterial and venous pressures, administer heparin when delegated or permitted by state law, obtain hemostasis, and apply dressings.
  • Obtain and document pre- and post-treatment vital signs, weights, treatment parameters, dialysis evaluations, and patient responses.
  • Initiate and discontinue dialysis treatments according to prescribed orders and established procedures, including blood-flow, dialysate-flow, ultrafiltration, and treatment-time settings.
  • Evaluate intradialytic problems and unusual patient conditions, provide prescribed interventions, and report significant changes or findings to the registered nurse or supervisor.
  • Calculate and enter patient-specific dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits 160, and conduct machine safety checks.
  • Obtain prescribed laboratory tests, collect and prepare specimens, prepare requisitions, label and store samples, and ensure collection by the laboratory courier.
  • Maintain a safe and clean environment by cleaning and disinfecting dialysis machines, chairs, equipment, and surrounding areas between treatments and promptly cleaning blood spills.
  • Prepare, organize, and use supplies and equipment efficiently; assist with supply ordering and inventory for the Transitional Care Unit.
  • Monitor patients performing self-care under registered-nurse supervision.
  • Assist in training applicable direct patient-care staff under the supervision of a nurse.
  • Enter treatment data into the designated clinical application and complete Hemodialysis Treatment Sheets and progress notes accurately and promptly.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and correct or obtain correction of omitted entries.
  • Prepare initial patient charts for admission and compile paperwork for chart placement.
  • Participate in Continuous Quality Improvement activities and infection-control audits, and reinforce infection-control practices and education for patients and families.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate transitional dialysis care.
  • Coordinate with appropriate staff to support patient experience and ensure disciplines are scheduled according to plan.
  • Explain home dialysis products and benefits, demonstrate peritoneal dialysis setup, and assist patients with understanding treatment options and transitioning to their chosen modality.
  • Set up and perform treatments on all machines used in the Transitional Care Unit.
  • Travel between assigned facilities and community locations and attend regional, business unit, or corporate meetings when required.
