- Review and process physician orders within the electronic medical record.

- Support intake and referral processing through clinical review as needed.
- Review discharge reports to ensure required documentation is complete.
- Process and lock transfer to inpatient facility and death-at-home Outcome and Assessment Information Set visits.
- Verify discharge summaries and signed orders are present.
- Confirm Notice of Medicare Non-Coverage documentation is completed when applicable.
- Process discharge-related orders as needed.
- Manage hospital-hold documentation and inpatient-transfer workflows.
- Assist with plan-of-care updates for hospitalized patients.
- Support resumption-of-care workflows and monitor pending approvals.
- Complete discharge workflows for patients who remain in facilities at the end of an episode when appropriate.
- Review infection-control reports and escalate concerns as needed.
- Monitor wound-score documentation and identify variances.
- Review quality-improvement events and communicate trends to leadership.
- Process clinical alerts and potentially avoidable-event notifications.
- Audit missed visits and related documentation to support quality-care delivery and reimbursement compliance.
- Monitor for timely initiation-of-care concerns.
- Review vital-sign alerts and ensure appropriate follow-up documentation is completed.
- Monitor coordination notes for physician updates and clinically significant medication-related concerns.
- Review electronic-medical-record dashboards for changes in patient condition and escalate issues appropriately.
- A multistate or compact Registered Nurse license is preferred.
- Willingness to seek additional state licensure is preferred.
- Previous home health management experience is preferred.
- Experience supporting multiple locations or clinical teams remotely is preferred.
