Pair Team connects vulnerable patients with local care teams that provide physical, mental, and social services. They achieve this by partnering with large healthcare providers, hospitals, health systems, municipalities, shelters, and social support services. The company focuses on whole-person care, offering their services for free to patients by coordinating care across their partner network. This model is funded through the Enhanced Care Management benefit of Medi-Cal, ensuring no cost to patients. Additionally, Pair Team compensates community partners for their involvement in patient care, creating a revenue stream for these organizations. They also provide extra support like virtual urgent care, free grocery delivery, and housing navigation, which enhances patient care and eases the workload for healthcare staff. Pair Team stands out by specifically targeting vulnerable populations and collaborating closely with community organizations to deliver comprehensive care.
