Requirements
  • A Bachelor's degree in Healthcare Administration, Public Health, Social Work, or a related field is required.
  • At least 2 years of experience in quality assurance, care coordination, compliance, or healthcare program auditing is required.
  • Familiarity with Enhanced Care Management, Community Supports, Behavioral Health, Community Health Worker, CalAIM, Medi-Cal, HIPAA, and care coordination workflows is required.
  • Strong analytical and problem-solving skills are required.
  • Excellent documentation, communication, and interpersonal skills are required.
  • Proficiency with customer relationship management and electronic medical record systems and Microsoft Excel is required.
  • A driver's license and reliable transportation are required for 40% community outreach.
Responsibilities
  • Audit member records, assessments, and documentation to ensure compliance with Enhanced Care Management, Community Supports, Behavioral Health, Community Health Worker, CalAIM, and Medi-Cal standards.
  • Monitor outreach, referral, and disenrollment workflows for adherence to required timelines and regulatory guidelines.
  • Ensure HIPAA compliance and data integrity across member-facing processes.
  • Evaluate Enhanced Care Management, Community Supports, Behavioral Health, and Community Health Worker workflows for accuracy, efficiency, and risk exposure.
  • Identify trends, documentation gaps, and systemic errors.
  • Develop corrective action plans and process improvements to enhance operational performance.
  • Track quality assurance metrics across documentation accuracy, outreach compliance, and verification standards.
  • Generate reports outlining findings, trends, risk areas, and performance improvements.
  • Collaborate with leadership to achieve Enhanced Care Management, Community Supports, Behavioral Health, and Community Health Worker quality assurance metric thresholds.
  • Partner with Behavioral Health, Community Health Worker, Community Supports, Enhanced Care Management, Outreach, and leadership teams to reinforce quality standards.
  • Provide feedback, coaching, and training based on audit findings.
  • Support implementation of new workflows and regulatory updates.
  • Conduct community-based outreach across multiple counties through in-person engagement and digital channels, including phone, SMS, email, and social media, to identify, engage, and enroll eligible members into programs.
  • Build and maintain partnerships to expand access to services in high-need communities.
