  • CPR certification (BLS or ACLS) is required.
  • A current and valid Drug Enforcement Administration (DEA) certificate and a valid Oregon driver’s license are also required.
  • For Nurse Practitioners: a valid, unrestricted license as an independent Oregon-licensed Nurse Practitioner (NP) with the Oregon State Board of Nursing (OSBN) is required; an active Oregon RN license is also required.
  • For Physician Associates: a valid, unrestricted license as a Physician Associate (PA) in the State of Oregon with the Oregon Medical Board is required; a PA collaboration agreement must be signed by an MD/DO/DPM physician or employer as defined in ORS 677.495 and OAR 847-050-0010, available at primary location and to Oregon Medical Board upon request.
  • Be legally authorized to practice in the State where performing the function, and only act within the scope of authority; have 1 year of experience working with a frail or elderly population; meet a standardized set of competencies for the specific position description established by the PACE organization; be medically cleared for communicable diseases before engaging in direct participant contact.
  • Travel is required approximately 20% of the time.
  • Minimum of 1 year of experience working with a frail or elderly population; 5 years of nursing or varied healthcare exposure and experience are necessary; experience in a value-based healthcare organization or payer model is preferred.
Responsibilities
  • Provide and manage direct participant care, including physical examinations, evaluations, assessments, diagnoses, treatment, and care planning.
  • Perform basic office-based, ambulatory procedures.
  • Participate in tele-health/virtual health practices as indicated.
  • Provide Age-Friendly and goal-concordant participant care.
  • Engage in Advance Care Planning (ACP) and palliative approaches to participants in the context of goal-concordant care.
  • Complete and sign reports of death in compliance with state law.
  • Actively participate as a member of the PACE Interdisciplinary Team to develop and implement a comprehensive plan of care for each participant upon enrollment and throughout their enrollment as specified by PACE regulations.
  • Collaborate closely with the PACE Nursing team in delivering care within the clinic and at participants’ homes.
  • Ensure clear and accurate documentation within the Electronic Health Record (EHR), including all communication and decision-making as specified by PACE regulations.
  • Advise on care management and examine patients with other PACE PCPs, including physicians, Nurse Practitioners, and Physician Associates (PA), as indicated.
  • Collaborate with specialists, including dentistry, podiatry, chiropractic, therapies (speech, physical, occupational), and medical subspecialties.
  • Coordinate care across all settings, including hospitals, skilled nursing facilities, and assisted living facilities, and closely collaborate with other attending physicians.
  • Visit participants in their homes or care settings as indicated.
  • Fulfill all administrative duties on a timely basis, including record-keeping in participant charts and appropriate and medically necessary diagnosis coding.
  • Conduct timely peer-to-peer discussions to clarify clinical information and explain review outcome decisions across the continuum of care.
  • Prescribe pharmaceuticals and treatment regimens as appropriate to assessed medical conditions.
  • Refer to and coordinate with specialists as appropriate.
  • Serve on various internal and external committees as required or designated.
  • Provide all services in a manner consistent with PacificSource core values and State and Federal regulatory requirements.
  • Participate in team meetings, caregiver groups, and special events as indicated.
  • Provide on-call coverage for direct patient care after hours and on weekends as indicated.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
Desired Qualifications
  • Experience in a value-based healthcare organization, payer model, or closely related field is preferred.
  • Prior experience in PACE, skilled nursing, or a managed Medicaid setting is preferred.

PacificSource grants $40,000 to support St. Peter's Health programs.

PacificSource grants $40,000 to support St. Peter's Health programs. St. Peter's Health Foundation has received $40,000 in grant funding from PacificSource to expand community programs that improve health and wellbeing for people experiencing housing insecurity and support access to healthcare services and items needed for healing across St. Peter's Health's five-county service area. The funding includes a $25,000 grant from the PacificSource Foundation for Health Improvement to support St. Peter's Health's Housing is Healthcare program and a $15,000 Community Health Excellence Grant to expand the St. Peter's Health Foundation Patient Assistance Program. Both programs rely on grants and community contributions to meet growing local needs, and PacificSource's investment helps sustain these critical services. The Housing is Healthcare program is led by St. Peter's Health Community Health Workers (CHWs), who work directly with people experiencing homelessness or housing instability to reconnect them with medical and behavioral healthcare, substance use treatment, Medicaid and other public assistance programs, and housing resources. Stable housing is foundational to health. By addressing the social and economic factors that influence wellbeing, the program improves long-term health outcomes and reduces avoidable emergency department visits for people with complex medical and social needs. In 2025, more than 60 people in the Helena community were supported through the program. The St. Peter's Health Foundation Patient Assistance Program helps remove barriers that can prevent people from accessing care and the essential resources they need to heal - items not covered by insurance or other programs and often unaffordable for patients. Support includes transportation assistance, gas cards, medications, medical supplies, and other items needed for recovery. In 2025 alone, the program provided $205,254 in assistance to 1,183 patients and their families across St. Peter's Health's five-county service area. "Access to care isn't just about doctors and clinics. When people have a way to get to care and the resources they need to heal and stabilize their lives, health outcomes improve and our whole community benefits. Programs like our Patient Assistance Fund and Housing is Healthcare are key solutions to these challenges. We're grateful to PacificSource for their support," said Wade Johnson. "At PacificSource, we believe that health starts long before someone walks into a clinic. Stable housing, transportation and access to basic resources all play a critical role in health and recovery. Through our support of St. Peter's Health's Housing is Healthcare and Patient Assistance programs, we're proud to invest in solutions that meet people where they are and strengthen the health of the Helena community," said Erik Wood, Enterprise Market President, PacificSource.
