Personal Support Worker

In this community support role, as a Personal Support Worker (PSW) you will have scheduled visits to care for patients with palliative care needs as part of a palliative care team comprising of Nurses, PSW’s and Allied Health members. You will work with patients of all ages with differing complexity of care and play a key role in helping patients navigate through their care journey.

What you’ll do:

• Assist patients with activities of daily living including skin, hair, and oral care along with bathing, toileting, & peri-care
• Liaise with other members of the client’s palliative care team
• Prepare nutritious meals and snacks, including special diets, as determined by the care plan
• Provide basic housekeeping tasks and companionship to help your patients enjoy a better quality of life

What you bring:

• Valid Personal Support Worker Certificate
• PSWs palliative care and PPS trained: Palliative Care Fundamentals & LEAP PSW Course (or will be trained within 3 months upon hire) preferred
• Ability to demonstrate compassion and care when interacting with patients
• Reliability and flexibility
• Passion for making a difference in your community
• Valid driver license and access to a vehicle to travel to and from patient’s residences within a geographical area
• Clear Vulnerable Sector Screen
