Responsibilities
  • Works with the Center’s physicians and other key personnel to operationalize grant deliverables, orchestrate change as indicated and ensure continuous quality improvement.
  • In conjunction with Center RN, participates in patient visits. Responsible for reporting the patient history and complaints to the physician.
  • Maintains confidentiality and adhere to HIPAA regulations.
  • Administers baseline and routine cognitive testing (MMSE, MoCA) as well as ADL/IADL, CDR neuropsychological inventory, AD8.
  • Provides counseling and case management services to patients and/or care partners to help them understand and accept their diagnosis and cope with the emotional stress and manage the day-to-day services.
  • Facilitates community service referrals if/when needed and complete appropriate paperwork that is requested for services.
  • Attends community meetings to strengthen collaborative partnerships and identify groups for potential trainings.
  • Development of a targeted outreach plan reaching primary care providers/medical specialty physicians to highlight the importance and benefits of early diagnosis and the role of the CEAD as a resource to the 10 counties of our grant catchment area.
  • Development of a targeted resource guide for caregivers on community-based providers/services for patient and family referrals in each of the 10 counties of our grant catchment area.
  • In conjunction with the Program Director, Program Manager and Center physicians, the Case Manager will participate in and conduct training programs for medical professionals that will include information about the diagnosis, treatment and care management of individuals with AD/D, the importance of early detection, management of behavioral issues, need to address palliative care and ethical issues facing someone with dementia and the importance of screening individuals over the age of 65 for cognitive impairment as a component of regular care using a validated cognitive screening tool.
  • As assigned by Program Manager, represents the CEAD at meetings, conferences and other events as required. Interface with any/all community agencies, which may serve to benefit the Center/CEAD or its constituency.
  • In conjunction with Program Manager, assists with maintaining consistent follow-up with primary care providers/medical specialty physicians, non-physician health care providers and college and universities (for student trainings) to strengthen relationships.
  • In conjunction with Program Manager, develop and maintain relationships with key personnel from Albany Med to identify training opportunities for Albany Med physicians and non-physician health care providers
