Requirements
  • A nursing degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • A current, unencumbered, active Licensed Practical Nurse/Licensed Vocational Nurse license in the state.
  • Current cardiopulmonary resuscitation certification.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines applicable to nursing care facilities.
  • Knowledge and experience with PCC is preferred.
  • Ability to read, write, speak, and understand English; read technical procedures and policy and procedure manuals; present information; and respond to questions from managers and employees.
  • Ability to apply fractions, percentages, ratios, and proportions to practical situations.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies and personnel, and the general public.

Responsibilities
  • Provide safe, quality nursing care to residents, including medication administration, treatments, assessments, documentation, and monitoring changes in resident condition.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Make written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total regimen of care.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures to personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports and maintain required census, medical, and clinical records.
  • Protect resident health information, user credentials, and facility information systems from unauthorized disclosure or access.
  • Complete recordkeeping forms and charts for resident admissions, transfers, and discharges.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nurses’ notes descriptively, document care and resident responses, and sign and date medical-record entries.
  • Prepare and administer medications and treatments as ordered by physicians, verify resident identity, maintain medication supplies, and report medication or narcotic discrepancies.
  • Order prescribed medications, supplies, and equipment as necessary and maintain accurate narcotic records.
  • Review medication cards for completeness and accuracy and notify physicians of automatic stop orders.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend employee dismissals or transfers to the Nurse Supervisor.
  • Report staffing shortages and absences, develop work assignments, and provide leadership to nursing personnel.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards and report problem areas.
  • Ensure nurse aide trainees are supervised by a licensed nurse and that nursing assistants are enrolled in or have graduated from an approved training program.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Review personnel complaints and grievances and report them according to facility procedures.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Inform nursing personnel about new admissions and prepare rooms for admissions.
  • Orient new residents and family members to the facility and make rounds with physicians as necessary.
  • Arrange diagnostic and therapeutic services as ordered and consult residents’ physicians regarding care, treatment, and rehabilitation.
  • Review resident charts for treatments, medication orders, diets, and other care requirements.
  • Implement and maintain nursing objectives and standards and check that prescribed treatments are properly administered.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin about resident accidents, incidents, or changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect nursing treatment areas and resident care rooms to ensure they are clean and safe.
  • Provide professional nursing services, including catheterization, tube feedings, suctioning, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massages, range-of-motion exercises, and care for dying residents.
  • Obtain sputum, urine, and other specimens for laboratory tests and record temperatures, pulses, respirations, and blood pressures.
  • Monitor seriously ill residents and check frequently on residents unable to call for help.
  • Ensure direct care follows resident care plans and residents’ wishes and address dietary concerns.
  • Arrange transportation and escort discharged residents, notify families of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training and orientation for nursing personnel.
  • Assist with clinical supervision of nurse aide trainees and attend required external, facility, continuing education, and advance-directive training.
  • Monitor compliance with safety, sanitation, infection-control, hand-hygiene, fire-safety, disaster-preparedness, and personal protective equipment procedures.
  • Maintain clean and sanitary work areas and ensure safe operation and disposal of nursing equipment and supplies.
  • Participate in development and maintenance of the infection-control program and procedures for reporting hazardous conditions.
  • Recommend equipment and supply needs, maintain adequate medication and medical-supply stock, and prevent waste.
  • Review resident care plans daily, identify needed revisions, and ensure nurses’ notes document care-plan compliance.
  • Assist with Minimum Data Set assessments, Resident Assessment Protocols, and care-plan coordination.
  • Provide data to the Quality Assurance and Assessment Committee and report suspected fraud related to false billings, cost reports, and kickbacks.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
