Requirements
  • Bilingual Spanish capability is preferred.
  • An active Licensed Practical Nurse state license is required.
  • A valid driver's license is required.
  • Two or more years of applicable nursing experience are required.
Responsibilities
  • Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes, including conducting patient home visits.
  • Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs.
  • Manage emergency situations in a prompt, precise, and professional manner.
  • Complete medication administration and treatment needs for all assigned patients.
  • Provide nursing care in accordance with applicable regulations and company policy.
  • Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
