Requirements
  • Bachelor’s degree in education, behavioral, social, or mental health, public health/administration, counseling, and/or social work required.
  • Minimum one-year experience conducting case management-related activities with the identified population.
  • Proficiency in Microsoft Office Programs is required.
  • Ability to use person-first recovery and resiliency language.
  • Ability to work with community members and community agencies to facilitate collaboration and cooperation.
  • Ability to establish relationships with those receiving services with compassion and respect.
  • Demonstrate excellence in follow-through and tenacity with hard-to-reach populations.

Responsibilities
  • Provide case management services and support for participants to increase the client’s ability to successfully navigate their lives as independently as possible.
  • Make home and community visits to assist consumers with access to, and engagement in, necessary mental health/substance use (MH/SU) services.
  • Care coordination between two or more agencies, including medical and non-medical providers; basic needs (housing, food and nutrition, transportation) or skill Development (including self, interpersonal, and crisis management).
  • Assist individuals in processes that facilitate their independence.
  • Assist individuals in completing activities to ensure that basic needs are being met.
  • Utilize the referral tracking and service criteria to initiate and monitor service referrals.
  • Utilize electronic medical records, including taking effective case-notes, tracking case management efforts, reporting NOMS surveys at the expected intervals, and outcome driven data collection (including participant self-report).
  • Organize mailings and make outreach phone calls to participants, local service providers, government agencies, non-profits, school administration and others.
  • Plan, facilitate and attend informational meetings and public events that will assist in supporting participants.
  • Work with local community service providers and agencies to build partnerships that benefit participants.
  • Participate in all required professional development and program meetings.
  • Maintain youth/family/individual confidentiality and obtain appropriate release of information.
  • Be willing to utilize a flexible schedule that may include evenings and weekends, meetings at schools, public places, or in participant homes.
Desired Qualifications
  • Qualified Mental Health Professional (QMHP) or Peer Support Specialist (PSS) preferred.
  • Experience in evidenced based practices.
