Requirements
  • Licensed Practical Nurse licensure in the state of employment.
  • Ability to conduct independent home visits.
  • Experience providing whole-person care to adults and seniors with high medical and social complexity.
  • Current Cardiopulmonary Resuscitation and Basic Life Support certifications.
  • Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Microsoft Outlook.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation.
  • Proof of liability and property damage insurance for the vehicle used.
  • A driver's license held for a minimum of five years.
  • No vehicle-related suspensions, driving under the influence, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or three or more moving violations within the last five years.
  • No more than two moving violations or one at-fault accident within the last three years.
  • Frontline workers must be fully vaccinated and provide records for validation; medical or religious exemptions may be considered with appropriate documentation.
Responsibilities
  • Partner with care team members to develop and implement patient-centered clinical plans.
  • Deliver wrap-around services through home visits and collaborative care alongside providers and Registered Nurses.
  • Perform clinical tasks in the field, including chronic condition support, wound care, phlebotomy, and medication review.
  • Engage hard-to-reach patients by bringing vital care and support directly to their homes.
  • Provide coverage for team members when needed to ensure continuity of care.
  • Contribute to improved patient outcomes, lower costs, and greater patient and staff satisfaction through the interdisciplinary care model.

ConcertoCare provides in-home medical and social care for seniors, combining in-person visits with virtual care to keep patients independent and at home. It serves Medicare, Medicaid, and Medicare Advantage beneficiaries with complex needs, coordinating with an existing primary care provider when available. Its services span in-home primary care, medical and behavioral health care, transportation, food support, caregiver assistance, advanced care planning, and palliative care, delivered through three program offerings and enhanced by proprietary care pathways, analytics, and virtual care technology. ConcertoCare operates as a risk-bearing provider, aiming to improve health outcomes and quality of life for vulnerable seniors by delivering the right care to the right patients, where they are best served: at home.
