Requirements
  • A Master of Social Work degree from a program accredited by the Council on Social Work Education.
  • One year of full-time social work experience in a health care setting or equivalent, including practicum placement experience as part of a social work training program and/or employment experience.
  • Within 90 days of hire, hold a Washington state credential as an Agency Affiliated Counselor, Licensed Social Work Associate - Advanced, Licensed Social Work Associate - Independent Clinical, Licensed Advanced Social Worker, or Licensed Independent Clinical Social Worker.
Responsibilities
  • Partner with patients, their families, and the health care team to address medically related social and emotional needs and provide support and advocacy affecting medical condition, treatment, recovery, and safe transitions.
  • Provide psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referrals.
  • Partner with patients, families, and the health care team to coordinate care across the continuum and improve care through effective resource utilization and transitions from acute to post-acute settings.
  • Provide specialty consultation to health care staff and teams regarding psychosocial aspects of patient care.
  • Provide psychosocial support to patients and families.
  • Develop and implement safe and timely discharge plans in collaboration with the multidisciplinary team.
  • Provide care coordination for patients with complicated social and medical histories.
  • Provide case management and/or counseling services in specialized programs.
  • Make timely referrals to hospital and community agencies, including referrals required by mandatory reporting laws for child and vulnerable adult abuse.
