Requirements
  • An M.D. or D.O. from an accredited medical school is required.
  • Board Certification in Rheumatology is required.
  • An active, unrestricted medical license is required; Ohio licensure or willingness to obtain it is required.
  • Completion of an ACGME-accredited Internal Medicine residency and Rheumatology fellowship is required.
  • DEA certification is required.
  • Basic Life Support certification is required.
  • Comfort practicing in virtual and in-person care environments.
  • Strong written and verbal communication skills and the ability to explain complex concepts clearly to patients and care teams.
  • Comfort working with patients from diverse backgrounds and varying levels of healthcare literacy.
  • Proficiency with electronic health records and healthcare technology platforms.
  • Ability to work independently while contributing to a collaborative, team-based care model.
  • Belief in a whole-person approach to chronic disease management.
  • Willingness to travel periodically to partner sites as needed.
  • Documentation of a negative tuberculosis screening conducted within the last 12 months.
  • Proof of a completed Hepatitis B vaccine series or positive blood titers from within the last 12 months.
  • Proof of a completed measles, mumps, and rubella vaccine series or positive blood titers from within the last 12 months.

Responsibilities
  • Conduct new patient evaluations and specialist consultations for patients with complex rheumatologic and autoimmune conditions, including inflammatory arthritis and systemic autoimmune disorders.
  • Diagnose, treat, and manage chronic rheumatologic conditions across virtual and in-person care environments.
  • Provide physician-level review and oversight of escalated clinical cases from advanced practice providers, clinical pharmacists, and care team partners.
  • Develop individualized treatment plans integrating medical therapy, lifestyle interventions, and whole-person care approaches.
  • Educate patients on disease processes, treatment pathways, and long-term management strategies.
  • Identify co-occurring or contributing conditions that may impact diagnosis, treatment response, or patient outcomes.
  • Participate in collaborative case reviews and interdisciplinary care planning.
  • Maintain compliance with Community Health Center, clinical, and regulatory requirements.
  • Collaborate closely with advanced practice providers, clinical pharmacists, care navigators, and health coaches to deliver coordinated patient care.
  • Provide clinical guidance, mentorship, and support to advanced practice providers and pharmacy partners.
  • Contribute feedback on workflows, care protocols, and patient experience initiatives.
  • Support a culture of clinical excellence, accountability, continuous improvement, and patient-centered care.
  • As the role progresses, manage a longitudinal patient panel with increasing accountability for patient outcomes, treatment strategy, and clinical leadership across the panel and care team.
  • Participate in growing and developing the patient panel through clinical relationships, collaboration with partners, and engagement with the communities served.
  • Participate in multidisciplinary care oversight, advanced practice provider collaboration and mentorship, partner engagement, and panel development activities.
  • Contribute to clinical program development, including protocol design, quality initiatives, and service line evolution.
  • Build relationships with Community Health Centers and health system partners.
