- The Nurse Practitioner must graduate from an accredited School of Nursing program and be licensed as a registered nurse.
- The Nurse Practitioner must be certified by graduating from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner.
- Other Nurse Practitioner concentrations or specialty tracks may be considered based on experience and business need.
- The Physician Assistant must graduate from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant and pass the Physician Assistant National Certifying Exam to use the Physician Assistant-Certified designation.
- Physician Assistants must hold state licensure and National Commission on Certification of Physician Assistants certification.
- Advanced practice registered nurses and physician assistants must have an unrestricted professional license and Drug Enforcement Administration registration in each state in which they practice.
- Report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
- Collaborate on-site with the Center Leadership Team regarding center-specific goals and priorities.
- Collaborate with the Medical Director and/or Attending Physician on patient-specific medical care.
- Provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
- Work collaboratively with Center Leadership to improve patient outcomes through high-quality health care and everyday compassion.
- A minimum of two years of experience working with adult or geriatric populations is preferred.
