Requirements
  • Must pass the Department of Human Services background check and maintain qualified status.
  • Must not be excluded from working in government health programs by the Office of Inspector General.
  • Must be able to successfully meet the company's physical job demands.
  • Must have a valid current driver's license and vehicle insurance as required by state law.
  • Must successfully complete a Medication Administration class with a score of 80% or greater and regularly pass medications using the correct procedure without error.
Responsibilities
  • Assist clients with daily care, activities of daily living, and/or mental health supports.
  • Assist individuals with identified areas of need, including hygiene, domestic tasks, leisure time usage, grooming, socializing, budgeting, money usage, safety and health, medical appointments, homework completion if applicable, and community participation.
  • Empower individuals to develop and maintain skills that promote independent living, with a focus on maximizing their health, safety, and engagement in daily activities.
  • Work with clients to plan and implement recreation, leisure, and community activities.
  • Drive to and from and accompany clients at appointments and community activities using an employee's personal vehicle or a company vehicle when available.
  • Assist supported individuals in establishing and maintaining community relationships and facilitating friendships.
  • Implement programs in accordance with the Support Plan, Support Plan Addendum, and Individual Abuse Prevention Plan.
  • Provide person-centered services through practice, action, and language.
  • Maintain working knowledge of each person's history, skills, vulnerabilities, support plans, and emergency procedures.
  • Participate in the support team process and assist with developing and implementing goals.
  • Administer and document prescribed medication, treatments, and diets according to the Support Plan and Support Plan Addendum or as otherwise directed.
  • Maintain confidentiality in accordance with HIPAA and MORA policies and procedures.
  • Communicate verbally and in writing with others at work, using the client's primary spoken language when working directly with clients.
  • Complete all mandatory training required by MORA or 245D regulations.
  • Read and follow company policies and procedures.
  • Adhere to processes and procedures related to Electronic Visit Verification.
  • Maintain a 95% utilization rate for worked hours, consisting of billable client hours, each month.
  • Document program data and client billing information accurately and in a timely manner.
  • Collaborate with the Community Supports staff team and complete other assigned or needed duties.
Desired Qualifications
  • At least 1 year of experience with clients with intellectual/developmental disabilities, traumatic brain injury and/or mental health needs.
  • Interest in supporting clients with intellectual/developmental disabilities, traumatic brain injury and/or mental health needs.
