- A high school diploma or equivalent is required.
- A valid New York, New Jersey, Connecticut, or Pennsylvania driver's license with at least six months of driving experience and six or fewer points is required.
- Strong interpersonal and communication skills, patience, empathy, and a genuine desire to support persons with developmental disabilities are required.
- The ability to respond effectively to changing needs and environments, demonstrating flexibility and problem-solving skills, is required.
- The employee must maintain confidentiality, uphold ethical standards, and follow agency policies and procedures.
- A strong commitment to safety is required.
- Implement and monitor varying plans of care for the persons supported, including Comprehensive Functional Assessments, Staff Action Plans, Life Plans, treatment plans, and Behavior Support Plans.
- Implement and document goals and objectives from person-centered plans, focusing on social, vocational, and life skills through structured activities and positive reinforcement.
- Provide direct care and assistance with daily living activities, including personal hygiene, meal preparation, medication administration, and transportation.
- Facilitate community outings, recreational activities, and social events to promote inclusion, independence, and meaningful interactions.
- Accompany individuals to appointments and healthcare follow-up.
- Use person-centered approaches and positive behavior support strategies to address challenging behaviors and maintain a safe, nurturing environment.
- Provide the required level of medical, developmental, clinical, and emotional supervision and support.
- Maintain accurate records, daily progress notes, and incident reports to track achievements, behaviors, and health status and ensure compliance with agency policies and regulatory requirements.
- Collaborate with healthcare professionals, therapists, and family members to develop and implement comprehensive care plans.
- Advocate for the rights, choices, and preferences of individuals with developmental disabilities and promote self-advocacy and empowerment in decision-making.
- Act as a custodian and mandated reporter in accordance with OPWDD, NYS Justice Center, and organizational policies.
- Administer medications according to prescribed schedules, document medication administration accurately, and monitor for adverse reactions or side effects.
- Experience in direct care or human services is preferred.
- CPR and First Aid certification are preferred.
- Additional training in developmental disabilities, behavior management, or healthcare is desirable.
- A genuine commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities is desirable.
