Requirements
  • A Bachelor's Degree or an equivalent combination of education and experience is required.
  • Graduation from an accredited School of Nursing as a Registered Nurse is required.
  • Current appropriate state licensure is required.
  • Six to eight years of related experience or an equivalent combination of education and experience is required.
  • At least 12 months of clinical nursing experience is required.
  • At least six months of chronic or acute dialysis nursing experience is required.
  • The employee must be available full-time and provide on-call coverage when necessary.
  • The employee must demonstrate leadership competencies and skills including communication, customer service, continuous quality improvement, relationship development, results orientation, team building, motivating employees, performance management, and decision making.
  • The employee must demonstrate management skills necessary to supervise dialysis personnel and ensure maximum quality care for patients.
  • The employee must complete Clinical Manager training modules and ongoing developmental programs within the specified timeline.
  • The employee must complete the Ishihara's Color Blindness test as a condition of employment, with reasonable accommodations considered if needed.
Responsibilities
  • Ensure quality patient care and cost-effective clinical operations in accordance with legal, compliance, and regulatory requirements.
  • Make daily clinical leadership decisions to ensure continuity of care and patient and staff safety.
  • Manage the tactical execution of short- and long-term objectives, coordinating activities and maintaining responsibility for costs, methods, staffing, and results.
  • Provide leadership, coaching, and development plans for direct reports, partnering with Human Resources on employee matters.
  • Support and drive FMS quality standards by meeting ESRD regulatory requirements and practicing Quality Assessment and Improvement using FMS QAI tools.
  • Address adverse events and action thresholds.
  • Oversee the facility's Home Therapies Program when applicable.
  • Ensure compliance with applicable federal, state, and local laws and regulations.
  • Communicate and implement FMS Clinical Quality policies and procedures for facility staff.
  • Maintain the integrity of medical records and FMS administrative and operational records.
  • Complete data collection and auditing activities.
  • Maintain facility environmental integrity and safety.
  • Coordinate all aspects of patient care with appropriate staff members from patient admission through discharge.
  • Educate patients regarding quality measures, transplant options, modality awareness, access care, catheter reduction, and adherence to treatment regimens.
  • Act as a patient resource for concerns and questions.
  • Organize interdisciplinary team meetings to complete patient care assessments and care plans and resolve patient problems.
  • Direct efficient and timely patient scheduling to maximize facility station efficiency.
  • Monitor each patient's situation, including hospitalizations, no-shows, catheter use, and significant changes in care status.
  • Develop action plans for unexcused and missed treatments in collaboration with the Director.
  • Coordinate patient care under the disease management agreement, including member eligibility validation, access management, dialysis services, patient education, hospitalizations, kidney transplantation, and communication with the multidisciplinary renal team.
  • Maintain current knowledge of disease management software and systems.
  • Implement FMS staffing and medical supply models and make recommendations.
  • Participate in recruiting, interviewing, and hiring decisions.
  • Ensure completion of new-hire orientation, training, mandatory in-services, ICD code training when applicable, and related documentation.
  • Ensure patient care employees receive training required by FMS policy and risk-management initiatives.
  • Provide professional growth and training opportunities so licensed staff can assume Team Leader responsibilities.
  • Oversee the performance of licensed personnel, direct patient care staff, assigned indirect patient care personnel, and technical staff when necessary.
  • Provide employee education, guidance, and performance feedback.
  • Provide regular counseling for clinical staff and encourage professional growth.
  • Complete timely employee evaluations and establish annual staff goals.
  • Document disciplinary conferences and confer with the Director and Human Resources regarding disciplinary decisions.
  • Participate in employee recognition and satisfaction programs.
  • Create, maintain, and communicate efficient employee schedules according to facility needs.
  • Create and implement a Continuous Quality Improvement Process Improvement Team involving staff in problem solving.
  • Facilitate physician privilege applications and compliance with FMS Medical Staff By-Laws.
  • Maintain strong Director and physician relationships and facilitate staff relationships with physicians.
  • Ensure regular and effective communication with physicians through meetings with Directors.
  • Participate in the Governing Body.
  • Schedule and coordinate Continuous Quality Improvement meetings with physicians.
  • Maintain the integrity and safety of the facility water system.
  • Maintain knowledge of facility equipment and technology operation.
  • Maintain and update FMS manuals.
  • Complete the Annual Standing Order Review and ICD coding.
  • Review correspondence and respond as appropriate.
  • Direct information gathering to support billing and collection activities.
  • Manage utilization of medication, laboratory, inventory, supplies, and equipment to achieve supply cost goals under FMS formularies.
  • Participate in completion and interpretation of the Pl-17 inventory supply use analysis.
  • Review and approve facility payroll.
  • Review profit and loss statements with the Director.
  • Participate in required Network reporting and on-site state or federal surveys.
  • Participate in completion of the FMS Administrative Clinical Review.
  • Travel between assigned facilities and community locations, and attend regional, Business Unit, and Corporate meetings when required.
Desired Qualifications
  • An advanced degree is desirable.
  • Experience in medical-surgical nursing or intensive care unit/coronary care unit nursing is preferred.
  • Three or more years of supervisory or project/program management experience is preferred.
