Requirements
  • A current Michigan license to practice as a registered nurse or physician assistant, as applicable, with specialty certification as a nurse practitioner and continued certification.
  • A DEA and Controlled Substances Certification/license.
  • All other LIP credentialing and privileging requirements, including maintaining at least Basic Life Support certification.
  • Thorough working knowledge of prevailing standards of care, InterCare clinical guidelines, patient care policies and procedures, and applicable accreditation and Patient-Centered Medical Home standards.
  • Thorough working knowledge of medical coding and documentation requirements.
  • Proficiency in an electronic health record.
  • Ability to use and respond to email regularly and use InterCare desktop tools, including incident reporting, the eLearning system, and ITAC.
  • Professional written and verbal communication skills.
  • Ability to communicate complex concepts clearly and effectively.
  • Ability to communicate professionally with patients, prospective employees, vendors, business associates, and staff members at all levels.
  • Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 pounds.
  • Good hand-eye coordination, reach with arms and hands, and finger dexterity, including the ability to grasp and manipulate a keyboard and office equipment.
  • Visual acuity to use a keyboard, operate office equipment, and read printed material, with the ability to talk and hear.
Responsibilities
  • Provide efficient and effective patient care according to adopted clinical standards relevant to the appropriate scope of licensure and certification.
  • Meet established peer review outcomes and daily productivity targets.
  • Meet patient satisfaction expectations and coding documentation standards in the electronic health record.
  • Engage fully in InterCare’s Patient-Centered Medical Home service delivery model.
  • Strive to achieve Uniform Data System expected outcome measures and Meaningful Use requirements.
  • Complete inbox and PAQ items within the designated timeframe.
  • Participate constructively as a clinical team member.
  • Communicate with InterCare’s Chief Executive Officer and Chief Medical Officer on matters of shared concern and provide input on pending matters upon request.
  • Meet staff satisfaction expectations.
  • Participate in assigned call schedule responsibilities.
  • Communicate with partners and the site manager regarding call schedules, time-off requests, and related coverage issues.
  • Attend provider, site, and assigned committee meetings as appropriate.
  • Read and respond to emails in a timely manner as outlined in policy.
  • Engage in practice improvement initiatives.
  • Participate in emergency response efforts upon request.
  • Care collaboratively for patients with the provider, Health Center Manager, and Director of Operations.
