Requirements
  • Bachelor's Degree in Nursing or similar field from an accredited college or university; or equivalent professional experience.
  • Licensed Registered Nurse (RN); multistate licensure preferred.
  • Five years of professional clinical, nursing, or related experience; virtual/telehealth experience preferred.
  • Understanding of broader clinical environment and alternative venues for care like telehealth.
  • Understanding of HIPAA regulations and privacy policies.
  • Experience using diverse technologies, such as: Trello, Slack, & Google Docs. Proficiency in Microsoft Office or equivalent applications required.
  • Able and willing to learn/adjust to changes in protocols and/or workflows.
  • Ability to manage multiple priorities and respond in a timely manner
  • Ability to execute under accelerated timeframes and quickly adapt to changes in priorities.
  • Excellent interpersonal skills.
  • Excellent telephonic communication skills. Effectively able to communicate with clinicians, colleagues and staff.
  • Skilled in developing, implementing, and administering work processes.
  • Strong organizational and time management skills; strong attention to detail.
Responsibilities
  • Support the clinical team in development of educational resources and care coordination protocols for patients.
  • Address patients' questions and concerns regarding their health, results, treatment plans, and prescribed medications via phone outreach.
  • Deliver lab results and provide information on treatment solutions.
  • Utilize standardized triage protocols and clinical judgment to prioritize patient care needs.
  • Refer patients to appropriate levels of care, including emergency services, primary care providers, or specialists, as necessary.
  • Document triage assessments and decisions accurately and comprehensively.
  • Ensure completion and reconciliation of patient intake information.
  • Facilitate medical documentation.
  • Provide follow-up with patients after consultation as appropriate.
  • Responsible for indexing of clinical patient records as requested.
  • Review & interpret immunization titers.
  • Document interpretation according to protocols.
  • Complete chart audits for process and health outcomes related to program goals as needed for the clinical team
