Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice, with one year of experience under the license.
  • Ability to collaborate with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Ability to work one-on-one with patients while receiving support from the patient care team.
  • Ability to work independently with phone, email, and written communication; meetings with healthcare professionals; and knowledge of hospice rules and regulations.
  • Time-management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple triaged concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide skilled nursing care and support to hospice patients during on-call hours.
  • Respond promptly to patient needs, triage calls, and make in-home visits as necessary.
  • Collaborate with the interdisciplinary team to ensure coordinated care.
  • Support patients and families through compassionate communication and clinical expertise.
