- An active Registered Nurse state license is required.
- A valid driver's license is required.
- At least two years of applicable nursing experience are required.

- Complete initial home assessments of clients, caregivers, and home environments for admission and continued services, determining eligibility and needs according to Department of Medical Assistance Services and agency regulations.
- Prepare written plans of care for administering care to clients.
- Conduct supervisory visits in clients' homes to ensure the quality and appropriateness of services and the clients' health, safety, and welfare.
- Provide necessary supervision to aides and clients and record significant documentation in client files.
- Observe, evaluate, and document the adequacy and appropriateness of care based on recipients' functional status and medical and social needs.
- Submit documentation requesting increases or decreases in the hours of care provided to members.
- Discuss client and family satisfaction with services during scheduled supervisory visits.
- Review aide records during supervisory visits in accordance with regulatory and organizational policies.
- Maintain timely and comprehensive clinical records, reports, and Department of Medical Assistance Services forms according to agency guidelines.
- Correct documentation as requested by the Clinical Supervisor.
- Coordinate client care with staffing coordinators and outside agencies or organizations as needed.
- Arrange referrals to physicians, skilled services, and other community agencies in collaboration with staff from other disciplines as needed.
- Maintain ongoing communication with paraprofessional staff regarding changes in client conditions or plans of care and evaluate care effectiveness.
- Advocate for clients and caregivers and report issues, concerns, or needed changes to the agency or support staff.
- Participate in clinical meetings and complete in-service training as directed.
- Maintain the schedule for required visits and complete visits according to regulatory and organizational criteria.
- Submit monthly Level of Care reviews according to Department of Medical Assistance Services guidelines.
