  • Graduate from a Specialty Training Program in Nursing.
  • Minimum of 2 years of work experience as a Licensed Practical Nurse.
  • Current Licensed Practical Nurse licensure from the appropriate issuing body.
  • Proficiency in computer literacy and the use of electronic medical records.
  • Ability to work a full-time schedule of 40 hours per week during day shifts.
Responsibilities
  • Perform duties as a patient navigator by guiding incoming calls and facilitating the entrance of new patients into the program.
  • Utilize clinical knowledge to determine actions necessary for patient introduction and evaluate patient needs via telephone or documentation review.
  • Collaborate with healthcare providers to coordinate and implement patient plans of care, including evaluation, treatment, and surveillance.
  • Communicate pertinent care information to patients via mail, telephone, and in person.
  • Ensure all necessary patient information is available for provider interactions.
  • Serve as a point of contact for providers and a liaison with scheduling services to ensure appropriate patient appointments.
  • Coordinate patient activities such as support groups and educational series.
  • Provide patient and family education regarding medical conditions and treatment plans.
  • Maintain systems and processes to ensure timely follow-up and surveillance of patients.
  • Develop and implement patient screening tools and efficient clinic navigational procedures.
  • Investigate patient concerns regarding appointment access, communication, and billing.
  • Conduct medical record reviews and report findings to physician specialists.
  • Provide direct nursing care and assist physicians during medical procedures.
  • Maintain clinical supply inventory and perform ongoing maintenance of exam and treatment rooms.
  • Inspect and observe the condition of medical equipment to ensure proper operation and identify needs for preventive or corrective maintenance.
