  • A Master's degree in Speech-Language Pathology is required.
  • State licensure as a Speech-Language Pathologist is required.
  • Strong communication and interpersonal skills are required.
Responsibilities
  • Perform ongoing clinical assessments, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and assess patient appropriateness for home care.
  • Develop and evaluate the plan of care in partnership with the patient, representative if applicable, and caregivers.
  • Prepare and complete clinical notes, phone orders, and other clinical record documentation on an ongoing and timely basis.
  • Analyze patient needs and provide appropriate physician-ordered care indicated in the plan of care.
  • Revise the plan of care based on evaluation and ongoing assessment data.
  • Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders using appropriate tests and measurements.
  • Provide services ordered by the physician as indicated in the plan of care.
  • Apply infection-control concepts and universal precautions while coordinating or performing patient-care activities to prevent contamination and disease transmission.
  • Accept clinical assignments consistent with education and competence to care for patients.
  • Communicate with physicians involved in the plan of care and other health care practitioners regarding the current plan of care.
  • Provide education to patients and caregivers.
  • Use interpersonal relations and communication skills and facilitate their use by other team members to achieve desirable outcomes.
  • Meet mandatory continuing-education requirements of the agency and licensing board.
Desired Qualifications
  • Experience in home health care is preferred.
  • Proficiency with Homecare Homebase software is a plus.
  • Dedication to patient-centered care.
