LifeLong Medical Care

View

LifeLong Medical Care provides medical and social services for uninsured people and patients with complex health needs, operating primarily as safety-net clinics across Alameda, Contra Costa, and Marin Counties. How its product works: it runs primary care and specialty medical services together with support services like case management and assistance accessing social resources, coordinating care to address both health and practical barriers. How it is different: it focuses specifically on uninsured individuals and those with complex needs within a defined regional network, offering integrated medical care and social support as a safety-net, rather than serving a broad insured population. Its goal is to improve health outcomes and access to care for underserved communities by delivering affordable, comprehensive healthcare and associated social services.
