  • Excellent customer service and communication skills

  • Prior working experience and understanding of health insurance or care coordination
Responsibilities
  • Make 60+ outbound calls per day to engage members, re-establish contact, and motivate them to complete important health actions (e.g., scheduling primary care visits, completing labs such as A1C or KED, optimizing prescriptions)
  • Use motivational interviewing and coaching techniques to influence and encourage behavior change, while maintaining empathy and respect for each member’s situation
  • Identify and resolve barriers to care, such as arranging transportation, coordinating Rx delivery, helping members access blood pressure cuffs or other health tools, or connecting them to community resources
  • Answer questions about the Wellth Rewards program and provide support for technical or app-related needs when members engage
  • Assist with healthcare navigation tasks, including finding providers, scheduling appointments, and arranging follow-up care
  • Re-engage members who have fallen behind on program check-ins or adherence goals
  • Communicate across channels (phone, text, email) with clarity, empathy, and persistence
  • Escalate complex or unique issues through the appropriate internal channels
  • Share feedback and insights with the team to continuously improve outreach strategies and member impact
  • Perform other duties as assigned
