- At least one year of recent Licensed Practical Nurse experience.
- An active New York State Licensed Practical Nurse license and current Basic Life Support certification.
- A physical examination completed within the past year.
- A PPD or QuantiFERON test completed within the past year, or a chest X-ray completed within the past five years.
- Measles, mumps, and rubella vaccination within the past ten years.
- Provide one-on-one patient care in the home setting.
- Work flexible 12-hour day or overnight shifts based on availability.
- Recent experience in acute care.
- A valid driver's license.
