  • Current registered nurse or licensed practical nurse licensure in the State of Oregon.
  • Valid CPR certification.
  • Strong clinical assessment and critical thinking skills.

  • Ability to work collaboratively with residents, families, providers, and interdisciplinary teams.

Responsibilities
  • Oversee and coordinate resident care planning and clinical services.
  • Conduct resident assessments and help develop individualized care plans.
  • Monitor resident conditions and communicate changes with physicians, providers, and interdisciplinary team members.
  • Review physician orders, test results, and treatment plans to ensure continuity of care.
  • Utilize electronic medical records to document and maintain accurate clinical information.
  • Communicate with residents and families regarding care plans, concerns, and updates.
  • Participate in clinical meetings and collaborate with nursing leadership and other departments.
  • Provide education and support to residents, families, and staff regarding care needs and treatment plans.
  • Support admissions, transitions of care, and discharge planning processes.
  • Ensure clinical documentation is accurate, complete, and compliant with regulations and facility standards.
  • Assist in supporting and guiding nursing staff.
  • Follow federal, state, facility, and company regulations and nursing standards.
  • Help maintain a positive, supportive, and resident-focused culture throughout the facility.
Desired Qualifications
  • Long-term care or skilled nursing experience.
  • Supervisory or leadership experience.
  • Six months of experience in a long-term care environment.
  • At least one year of supervisory or leadership experience in a healthcare setting.
  • Bachelor of Science in Nursing degree.
