Requirements
  • A current pharmacist's license issued by the Oregon State Board of Pharmacy.
  • Training or experience in a specialized area may be required at the time of recruitment.
  • A preferred BS in Pharmacy or Pharm.D. plus practice residency or equivalent experience, or specialist certification such as BCPS or BCOPS.
  • Preferred PGY-2 specialty residency and BLS/ACLS certification.
  • Preferred vaccination certification and teaching certificate.
  • Ability to meet pharmaceutical care needs across acute care, psychiatric, rehabilitation, skilled nursing, long-term care, obstetric, oncology, chemically dependent, HIV, home care, and hospice patient-care settings.
  • Experience with transitions of care, discharge medication education, surgical or internal medicine services, pain management, and medication reconciliation.
Responsibilities
  • Identify, resolve, and document complex therapeutic problems for the relevant patient population.
  • Respond to drug-information requests from physicians, nurses, patients, and families by reviewing and interpreting literature and applying it to patient-care situations.
  • Monitor therapeutic responses and medication adverse effects, consult providers about problems, document activities, and report adverse drug reactions, drug defects, and medication errors.
  • Supervise technicians, students, interns, and residents and monitor and evaluate their performance.
  • Form peer-level relationships with attending medical staff and serve as a drug authority in the practice area.
  • Assist in developing treatment guidelines for patients with specific diagnoses.
  • Document clinical decisions and medication-distribution issues in the patient medical record, pharmacy monitoring profile, drug-use evaluation data, or adverse-drug-reaction reporting program.
  • Interpret and evaluate physician orders and prescriptions for accuracy, completeness, and appropriateness.
  • Verify the accuracy and appropriateness of medications prepared and dispensed from the central pharmacy.
  • Participate in intra- and interdepartmental education programs and contribute to peer development.
  • Assist with medication reconciliation, educate patients about home medication regimens, and coordinate discharge medication plans with the interdisciplinary team.
  • Provide inpatient medication distribution and use services for patients across the lifespan, primarily adults and older adults on medicine inpatient wards.
  • Participate in patient rounding, transitions of care, order verification, pharmacy consultations, and medication-distribution coordination.
Desired Qualifications
