Requirements
  • One year of experience working with the disabled-adult or underserved population.
  • Lived experience in the community served and/or with client experience.
  • Detail-oriented with successful organizational skills.
  • Strong ability to communicate clearly and effectively.
  • Bilingual English/Spanish capability.
Responsibilities
  • Initiate telephonic contact with newly eligible enhanced care management potential clients after authorization is received from the health plan.
  • Complete face-to-face visits to enroll clients if they agree to a home visit.
  • Engage in culturally and linguistically appropriate client services as part of the community served by the program.
  • Provide education to potential clients and community organizations about the program and services provided.
  • Obtain consent to receive services and enroll new clients.
  • Maintain contact with new enrollees until engagement with the lead care manager.
  • Ensure accurate and timely documentation of all outreach activity in the program’s electronic health record.
  • Maintain open communication with the supervisor to track referral and enrollment statuses across programs.
  • Perform other reasonably related jobs as assigned.
Desired Qualifications
  • More than one year of experience working in home health care coordination and community-based organizations working with at-risk populations.
  • Experience working with health plan organizations and/or community case management programs.

Institute on Aging (IOA) provides services and advocacy to help Bay Area seniors maintain independence and safety as they age. It runs home care and case management, Elder Abuse Prevention, The Friendship Line (a 24-hour suicide prevention warm line for seniors), adult day care for those with Alzheimer's and dementia, and the Program of All-Inclusive Care for the Elderly (PACE), which combines medical care, physical therapy, and enrichment activities. IOA also offers CONNECT, a free community referral service for elder needs. Operating in San Francisco and surrounding regions (SF, Marin, San Mateo, Santa Clara, East Bay, the Peninsula, and parts of Riverside/San Bernardino), IOA differentiates itself through its integrated, community-based approach that combines direct care, crisis support, preventive programs, and navigation services to help older adults stay independent safely at home as long as possible.
