Requirements
  • Work within the scope of practice for a Hospice Aide under registered nurse supervision.
  • Follow verbal, written, and electronic medical record mobile-device instructions.
  • Hold state certification as a Certified Nursing Assistant, preferably, or qualify as a Home Health Aide.
  • Successfully complete the required Home Health/Hospice Aide competency evaluation under state and federal Medicare Conditions of Participation.
  • Hold CPR and First Aid certification, or obtain certification before starting employment, and keep it active during employment.
  • Have reliable transportation, a current state driver’s license, valid vehicle insurance, and hospice-required liability insurance.
  • Be fully vaccinated against COVID-19, or obtain a religious or medical exemption upon hire.
Responsibilities
  • Provide direct care to hospice and long-term care patients in their primary residences according to the Hospice plan of care.
  • Help maintain and restore patients’ physical, social, spiritual, and emotional health.
  • Provide care and support to patients and families during crises and end of life.
  • Deliver patient care including bathing, dressing, nail care, skin care, assistance with elimination and nutritional needs, mobility transfers, homemaking, maintaining a clean and safe environment, and taking vital signs according to the care plan.
  • Observe changes in patient conditions, document and report when indicated, recognize emergencies, and respond appropriately.
  • Work in diverse social and economic situations while maintaining appropriate boundaries and respecting patient and family values.
  • Represent Keystone Hospice positively in the community and provide customer service.
  • Follow agency, state, and federal policies, procedures, laws, and regulations relevant to the role, including Medicare Hospice and Long-Term Care regulations and HIPAA privacy standards.
  • Travel between long-term care facilities and patients’ primary residences.
  • Attend biweekly interdisciplinary team meetings when requested.
Desired Qualifications
  • At least one year of Certified Nursing Assistant, Home Health Aide, or Hospice Aide experience.
  • Hospice, home health, or long-term care experience.
