- Licensed Vocational Nurse with two years of geriatric nursing or assessment experience.

- Provide resident validation and effective redirection, anticipate residents’ needs, and communicate effectively with cognitively impaired residents.
- Order and maintain medications.
- Maintain accurate medical records.
- Schedule necessary resident services on a daily basis.
- Coordinate with physicians and health agencies regarding Physical Therapy, Occupational Therapy, laboratory work, and other special services.
- Assist with scheduling doctors’ appointments and transportation requests when necessary.
- Take proper action during emergencies in accordance with the approved emergency manual and notify designated personnel.
- Complete and email shift reports.
- Attend mandatory facility in-service programs and required management meetings.
- Attend Residential Health Care and Special Care Residence department meetings as scheduled.
- Attend ongoing training sessions scheduled by the Director of Staff Development and Director of Assisted Living/Special Care Residence.
- Assist in the effective operation of the facility and participate in relevant facility activities.
- Assist the Director in evaluating Special Care Residence Certified Nursing Assistants.
- Coordinate resident discharges from the Health Center back to the Special Care Residence and schedule follow-up care.
- Contribute to a positive, healthy, and safe working environment.
- Perform other relevant duties assigned by managers.
- One year of dementia care experience.
