  • A degree in a nursing field as a licensed practical nurse.
  • A current Licensed Practical Nurse license in good standing in the state or states where the employee practices.
  • Preferred: 12 months of facility or agency nursing experience as a Licensed Practical Nurse.
  • A valid driver's license, automobile insurance, and qualification to drive under the organization's motor vehicle check.
  • Good verbal and written communication, comprehension, computer, and interpersonal skills.
  • The ability to speak, read, write, and understand English.
  • Completion of required annual education, including Compliance, Privacy, Security, and HIPAA training.
  • Adherence to applicable organizational policies, procedures, codes of conduct, facility handbooks, and federal, state, and local laws and regulations.
Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
  • Implement the plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Assess patients and provide patient and family or caregiver education and information pertinent to the diagnosis and plan of care.
  • Participate in coordinating home health services and report identified needs for other disciplines to the registered nurse, Registered Nurse Case Manager, or Director of Clinical Management.
  • Provide mentoring, coaching, and education to direct care staff under the supervision of the registered nurse.
  • Use equipment and supplies effectively and efficiently.
  • Participate in on-call rotation.
  • Participate in interdisciplinary team meetings to coordinate client and family care, exchange information, problem-solve, and receive staff support and education.
  • Collaborate in policy creation and appropriate problem-solving methods.
  • Provide care with respect and dignity for residents or clients, their families, and staff.
  • Report resident or client and family complaints to appropriate supervisors and report allegations of abuse, misappropriation of funds or resident property, and other compliance concerns.
  • Perform other duties as assigned.
