  • Bachelor's degree or equivalent work experience
  • Two or more years’ experience working with frail or elderly disabled adults and caregivers
  • 2-3 years Inside Sales, call center, telephonic outreach, customer service or other similar experience
  • 1-3 years’ medical field experience and/or case management, home health
  • The ability to exercise judgement independently and voice noticeable issues or trends
  • Strong verbal and written communication skills
  • Strong problem-solving skills
  • Excellent time management, critical thinking and active listening skills
  • Possesses a high level of empathy and emotional intelligence
  • Strong proficiency in computer software navigation; Salesforce experience, as plus
  • Comfort in a dynamic and always evolving startup environment
  • Always maintain patient confidentiality; understands and adheres to HIPAA guidelines
Responsibilities
  • Schedule and perform initial home assessments
  • Serve as a field expert to support waiver member and caregiver onboarding experience
  • Perform required “home inspections” to ensure safety and compliance
  • Partner with Care team to monitor Daily Notes submitted by the caregiver
  • Maintain contact with the caregiver based on the needs identified during the initial assessment
  • Conduct annual training for each caregiver reflecting the waiver members assessed needs
  • Visit waiver member and caregiver to establish communication between all members of the care team
  • Partner with Care team in transitioning newly onboarded waiver members
  • Develop Care Plan for review by staff RN
  • Communicate concerns regarding waiver member or caregivers’ health, surroundings, safety, or unmet needs
  • Track all activities through entries into CRM
  • Function within the timeframes of a fast-paced startup in the healthcare environment
  • Perform other duties as required or assigned
