Requirements
  • Candidates with dual license in Maryland and DC or Maryland and Virginia will have preference over candidates who are only licensed in Maryland.
  • The managers will not adjust the work schedule that is discussed in this requisition.
  • These positions have the potential to be remote after training for candidates who reside in Virginia.
  • The candidate must be comfortable in a cubicle environment, able to type and talk at the same time at a conversational pace and able to navigate through multiple systems.
  • We are requiring that candidates start the process to obtain their DC and/or Virginia licenses within 30 days of completing training; the client will pay for the license.
  • Must be computer literate and comfortable with Microsoft Office; able to send and receive emails, attach documents, accept meeting invites, work in Word and Excel and be able to sort and filter data; Not a lot in PowerPoint; Heavy IM and email users; must be able to type and talk at the same time.
  • For selected candidates there will be a 30 minute telephone interview.
  • The license needs to be Maryland; however, candidates who hold licenses in Virginia and/or DC in addition to the Maryland license are also sought.
  • Licensed as a LCSW-C or LCPC or LCMFT.
Responsibilities
  • Provides care coordination to members with behavioral health conditions identified and assessed as requiring intensive interventions and oversight including multiple, clinical, social and community resources.
  • Provides assistance to members with questions and concerns regarding care, providers or delivery system.
  • Maintains professional relationship with external stakeholders, such as inpatient, outpatient and community resources.
  • Generates reports in accordance with care coordination goals.
  • Complies with Case management Society of America Standards for Case Management Practice and with CCMC code of Professional Conduct for Case Managers.
  • Assists with orientation and mentoring of new team members as appropriate.
Desired Qualifications
  • Preference for candidates with dual licensure in Maryland and District of Columbia or Maryland and Virginia.
  • Willingness to obtain additional licensure (DC and/or Virginia) within 30 days of completing training; client will provide payment for license.
  • Remote work potential after training for candidates who reside in Virginia.
